We would like to thank you for choosing Euphoria Medspa for your wellness and aesthetic needs. As one of our clients, we would like to keep you informed of the current office and financial policies.

BOOKING:

TTo schedule an appointment, feel free to give us a call at 909-212-0280. When booking any service, a $50 deposit is required, which will be applied to the services during your initial visit. For our valued members, no deposit is needed, but we kindly ask to have a credit card on file. Secure your spots wisely! If you have multiple services lined up and change your mind on the day of treatment can result in a service overbooking fee – we'll chat about that when the time comes.

At Euphoria Medspa, we uphold this deposit policy to ensure we maintain the highest quality of service for you. If you choose to use a third-party service to book your appointment, please note that the same policies outlined in this agreement will still apply.

We want to remind you that while our reception and non-medical staff are happy to help with general inquiries about our treatments and services, their responses should not be considered as medical advice. For personalized medical advice, it's always best to consult with Dr. Guevara-Channell or another qualified health professional during your appointment. We appreciate your understanding and cooperation, and we look forward to providing you with excellent care during your visit to Euphoria Medspa!

Cancellation/ Rescheduling / No Show Policy

We totally get that things pop up unexpectedly. But when appointments are forgotten or canceled without giving us a heads-up, it affects our ability to offer that time slot to others eagerly waiting for our services. That's why we've created these guidelines:

Cancellation

If you cancel an appointment with less than 24 hours' notice, your $50 non-refundable deposit is forfeited. For our members or package holders, the fee will be deducted from your account.

When you book multiple services in one go, it takes longer, right? So, we'd need a 20% non refundable deposit upfront and a 48-hour notice for any changes. But don't worry, if you need to reschedule and let us know before that 48-hour mark, your deposit becomes a credit for your next visit. If not, unfortunately, it's lost.

No Shows

Now, 'no shows' - they're a bummer for everyone. If you don't make it and miss your appointment, you will forfeit your deposit or you will be charged accordingly.

Same day appointments made within a 24-hour window are non-refundable.

Rescheduling

Rescheduling's cool, but hey, give us a heads-up at least 24 hours in advance. If you don't, that appointment deposit is a goner. We totally understand, but late reschedules throw a wrench in our plans just like cancellations, making it tough to ensure we have availability for people waiting to see us. Should there be a pattern of multiple reschedules exceeding three instances within a specified timeframe, it may impact our ability to continue accommodating the appointment or may result in forfeiture of the deposit and/or refusal of service.

Late Arrivals

We recommend being here 10 minutes early before your scheduled appointment time to allow for check-in and relaxation, and to ensure the full duration of your treatment. In the event of a late arrival, we will do our best to accommodate you within the remaining time. However, please note if there is not sufficient time to complete your visit you will forfeit your appointment deposit. Euphoria MedSpa also reserves the right to cancel an appointment of any client that has not arrived for their scheduled start time, without a refund.

APPOINTMENT REMINDER POLICY:

At Euphoria Medspa, we believe in making your experience as seamless as possible! Expect friendly reminders – we'll send you a text and/or email 1 week, 36 hours, and 24 hours before your scheduled service.

Just in case our reminder system decides to take a day off, we want to remind you that managing your appointment and sticking to our cancellation policy is still your responsibility.

GUESTS/PETS:

No Plus-Ones: Sorry, no room for extra guests, fur babies, or tiny humans during your glow sesh. We're juggling sharp tools and magic potions, and let's be real, accidents aren't on our agenda. Bringing them along could mean losing that deposit – no one wants that.

Guest Hijack Alert: Guests tend to go full Q&A mode, stealing your spotlight. If they need our wisdom, suggest they schedule their own visit with us alongside yours so we can have adequate time for the both of you.

Pet Pals: Furry friends and appointments don't mix. Pets might be cute, but they'll have to chill at home to keep everyone safe. Bringing them along could mean losing that deposit – no one wants that.

VIP Lounge: Our lounge area is an exclusive hangout for booked clients only. Sorry, squad, your golden ticket is a confirmed appointment.

IDENTIFICATION VERIFICATION:

We uphold a commitment to ensuring the safety, security, and personalized service for each of our valued clients. Upon Arrival: Clients will be requested to provide identification upon arrival at our facility. This step is essential to verify the identity of the individual receiving the scheduled treatment. Valid identification assists in preventing unauthorized access and protects against potential security breaches. Accurate identification allows us to maintain precise and up-to-date medical records for every client. Verification of identity is an integral part of confirming the consent process. It ensures that clients are fully aware of the treatments they are undertaking and have provided informed consent, aligning with industry best practices. In addition privacy laws like the Health Insurance Portability and Accountability Act (HIPAA) require healthcare providers to implement measures to protect patient information and maintain the confidentiality of medical records. We appreciate your cooperation and understanding in adhering to our Identification Verification Policy. If you have any concerns or inquiries regarding this policy, please feel free to reach out to our staff, who will be happy to assist you.

FINANCIAL POLICY:

Payment at Service: Payment is due at the time of your service. For some specific services, an advance deposit may be necessary. We accept cash, Visa, MasterCard, American Express, Discover, Cherry Financing, Care Credit, and Euphoria gift cards. Please note, gift certificates are non-redeemable for cash and are not replaceable if lost. They remain valid for one year from the purchase date.

Care Credit Benefits: Opt for Cherry/Care Credit to enjoy up to 6-12 months of 0% interest, depending on your expenditure. Ensure you have two forms of ID if using Cherry / Care Credit, and the account must be in your name, or we require the account owner's consent. Interested in a financing? Feel free to apply through the provided link.

Aesthetic Procedures Responsibility: Aesthetic treatments fall under elective procedures, making payment your responsibility. Insurance won't cover these services, even if you believe they might. Any additional expenses incurred outside our facility, including dissatisfaction with treatment outcomes, become solely your financial responsibility. Should a touch-up treatment be requested or required, you will bear the full cost. Feel free to request a price quote before undergoing treatment. Payment in full is mandatory at the time of service and is non-refundable.

Credit Card Authorization: By paying with a credit card, you confirm your status as an authorized user, granting us permission to charge the agreed-upon amount. In the event of a declined or reversed charge, you agree to cover a service charge and reimburse us for reasonable collection costs. Presenting any unauthorized payment method may be treated as fraud, potentially leading to law enforcement involvement and prosecution.

Third-Party Payment Details: If someone other than yourself is covering your treatment expenses, we require full contact information and a copy of their ID. In such cases, both you and the paying individual accept the complete financial and office policies outlined in this agreement, acknowledging the absence of refunds.

Dispute Resolution Agreement: Agreeing not to create or pursue a dispute or chargeback with your credit card company is crucial. By making a payment, you acknowledge that it is collected in good faith in exchange for a service or product you willingly elected, without any force or coercion. Attempts to leverage negative reviews or legal action for a refund will be treated as extortion and pursued legally to the full extent.

RESULTS:

Our commitment is to craft personalized treatments that cater to each patient, steering clear of a one-size-fits-all approach. While we uphold the highest standards in executing all treatments and procedures, we recognize that outcomes may not always align with every patient's expectations.

It's crucial to note that achieving full correction is our priority, and subsequent touch-ups or treatments might be necessary to sustain the desired effects. In the realm of non-surgical procedures, outcomes are not guaranteed, and patients willingly undertake these procedures at their own risk, as outlined in written consents and any verbal discussions between the provider and the patient.

While positive results are anticipated, the potential for complications, their nature, and the success or other results of treatment cannot be definitively foreseen or assured, either explicitly or implicitly. We inform our patients to expect potential bruising or swelling, emphasizing that variations in these experiences are normal.

Please be advised that results and the duration of treatment effectiveness hinge on various factors like age, sex, tissue condition, overall health, lifestyle, and sun exposure. Consequently, clinical outcomes will differ among patients, and a set duration for any treatment outcome cannot be guaranteed.

Before and After photos serve as visual aids to illustrate treatment progress, although they aren't a direct measure of actual results. They are intended to act as a reference for tracking progress.

Prior to undergoing a service, carefully contemplate all required protocols and potential side effects. Adhering to pre and post-care instructions is vital for the success of your desired outcome. Our commitment to client satisfaction extends to addressing any queries or concerns you may have about our services before making a purchase.

SERVICE RETURN POLICY:

All treatments and packages are non-transferable and non-refundable and expire one year from the purchase date. Any unused services in your package will not be refunded.

MEDICAL GRADE PRODUCTS RETURN POLICY:

Our medical-grade suuplements and skin care products have been curated by top leaders in the industry and specially formulated to meet the standards we desire for our patients. We feel they are amazing and we think you’ll enjoy them just as much. We do not accept any returns or exchanges on any products for any reason. Some exceptions may apply. Please call our office with any issues.

PRICES & PROMOTIONS

Pricing is custom, as each procedure and beauty plan is tailored by Dr. Guevara-Channell and/or her staff. A custom quote will be provided to you during the time of consultation.

We are committed to continuously expanding our services to ensure we bring you the latest and greatest technology. Although we make every effort to keep our website, spa menu, and/or flyers updated accordingly, please note that prices, services, offers, specials, and products are subject to change at any time without notice. Special offers and discounts may not be combined. When presented with more than one discount opportunity, we will automatically give patients the discount of greater value at the time of purchase.

By purchasing a service using a discount, you are in exchange agreeing to have your treatment filmed/photographed.  Your filmed content and before & after photos are to be used at our discretion for advertising and promotional purposes on our website and social media outlets. Also, promotions and discounted packages can/will be scheduled with our mid-level providers. Should you require Dr. Guevara-Channell to perform your treatment, the price can/will be different. All services performed by Dr. Guevara-Channell will be customized and priced will be given before or at the time of your visit.

INFORMED CONSENT / CONSENT FORMS:

By electively attending your appointment and receiving a treatment by one of our providers, you are responsible to inquire about any potential risks and/or side effects that may occur if you are unaware of them. You will have pre and post care instructions available to you before your appointment and it is your responsibility to request them in the event that you did not receive them. In most cases, written consents are provided to you in order to inform and warn you of possible risks, side effects, alternatives, etc…

A verbal discussion between you and your provider just before your treatment also serves as informed consent and warning in which you understand that you are electively undergoing the discussed procedure and are making a conscious effort to go forward with your treatment regardless of a signature.

Any consent form signed after a treatment has been performed also supersedes any lack of prior signed consent forms and serves as a consent for future treatments rendered by our clinic. 

Providers at Euphoria Medspa will only recommend treatments and procedures that will benefit you specifically and which are medically appropriate for you. It is your responsibility as a patient to disclose a full and accurate personal medical history including any and all information regarding medical conditions and my use of medications, drugs, herbs, vitamins, or other supplements of any kind. Failure to do so may affect your treatment outcome and increase the likelihood or severity of complications. Failure to disclose accurate information about my history may be grounds for dismissal from the Clinic. You must disclose if you are pregnant, trying to become pregnant, or breastfeeding to your provider. You must not be under the influence of illicit drugs or alcohol at the time of your visit.

PRESCRIPTIONS:

All patients are required to have an appointment with Dr. Guevara-Channell in order to obtain a prescription. A medical evaluation must be performed and if deemed necessary that you require a prescription medication. All prescribed medications must be monitored on a monthly basis in order to receive any prescription refills. We do not prescribe medications blindly, even if you have been on medication for several months. Things can change, and regular monitoring is necessary to avoid any wrongful use of medications.

RIGHT TO REFUSE TREATMENT:

Euphoria Medspa has the right to refuse treatment and/or dismiss a client from any service at any time. This can be a result of but not limited to excessive scheduling issues, disruptive behavior, mistreatment of our staff, and false expectations from treatments provided. In some instances, you may not be a candidate for certain medical services, and it is at the full discretion of the medical provider to determine whether you are a candidate for any service provided.
